چکیده

BACKGROUND Coronary arteriography is still widely accepted as a gold standard for the diagnosis of coronary artery disease (CAD), despite emerging methods such as multi-slice computed tomography. None of the presently available non-invasive diagnostic tests is perfect. The aim of the article was to make a comparison of the value and limitations of history, resting electrocardiography, exercise electrocardiography and dobutamine stress echocardiography in the diagnosis of CAD, and to create a simple algorithm for non-invasive diagnosis of CAD to optimize indications for coronarography. METHODS Prospective, multicentre trial. The collection of clinical data, resting electrocardiography, exercise treadmill electrocardiography, dobutamine stress echocardiography and catheterization data was performed on 600 patients with chest pain regarded as angina pectoris and no previous history of myocardial infarction. CAD was defined as >/= 50% narrowing of at least one major vessel. Final results were obtained in 551 patients, 65% male. The studied population was divided into three groups on the basis of pre-test likelihood of CAD: 1. high (> 70%), 2. intermediate (10-70%) and 3. low (< 10%). RESULTS Sensitivity and specificity of resting electrocardiography, exercise treadmill electrocardiography, dobutamine stress echocardiography and created algorithm were calculated: 23%, 87% and 93%, 21% and 85%, 69% and 96%, 44%, respectively. The prevalence of CAD in the studied population was 61%. CONCLUSIONS The diagnostic value of resting electrocardiography in stable CAD is low. Dobutamine echocardiography has comparable sensitivity but significantly higher specificity than exercise treadmill test. Our algorithm is simple, reasonably cost-effective and may be useful in decision making. When the probability of CAD is high, non-invasive testing is not indicated before coronary angiography; when it is intermediate or low, a first choice test should be different in female (stress echocardiography) and male (exercise electrocardiography). Diagnosis of Coronary Artery Disease via a Novel Fuzzy Expert System Optimized by Cuckoo Search

In this paper, we propose a novel fuzzy expert system for detection of Coronary Artery Disease, using cuckoo search algorithm. This system includes three phases: firstly, at the stage of fuzzy system design, a decision tree is used to extract if-then rules which provide the crisp rules required for Coronary Artery Disease detection. Secondly, the fuzzy system is formed by setting the intervals ...

Diagnosis of Coronary Artery Disease using Neuro-fuzzy-based Method

Background & Aim: Coronary artery disease is one of the most common diseases in different societies. Coronary angiography is established as one of the best methods for diagnosis of this disease. Angiography is an invasive and costly method. Furthermore, it is associated with risks such as death, heart attack, and stroke. Thus, this study introduces a neuro-fuzzy-based method which can help the ...

GENERALIZED FLEXIBILITY-BASED MODEL UPDATING APPROACH VIA DEMOCRATIC PARTICLE SWARM OPTIMIZATION ALGORITHM FOR STRUCTURAL DAMAGE PROGNOSIS

This paper presents a new model updating approach for structural damage localization and quantification. Based on the Modal Assurance Criterion (MAC), a new damage-sensitive cost function is introduced by employing the main diagonal and anti-diagonal members of the calculated Generalized Flexibility Matrix (GFM) for the monitored structure and its analytical model. Then, ...

بررسی شیوع هم‌زمانی عوارض میکروواسکولار و بیماری عروق کرونر در بیماران مبتلا به دیابت تیپ II

Abstract Abstract: Diabetic Mellitus (DM) is a systemic disease that affects all body organs. Micro and macro vascular complications progress with diabetes progression. It is important to find a solution for early diagnosis of coronary artery disease that is a major cause of death in these patients. The goal of this study is to assess the relation between diabetic nephropathy and retinopathy to...

Cost-Effectiveness of Different Diagnostic Strategies in Suspected Stable Coronary Artery Disease in Portugal

BACKGROUND Cost-effectiveness is an increasingly important factor in the choice of a test or therapy. OBJECTIVE To assess the cost-effectiveness of various methods routinely used for the diagnosis of stable coronary disease in Portugal. METHODS Seven diagnostic strategies were assessed. The cost-effectiveness of each strategy was defined as the cost per correct diagnosis (inclusion or exclu...
